Modern Healthcare and Architecture & Governance highlight collaboration using AI to scale care beyond traditional healthcare facilities.
Resilient Healthcare and Community Hospital Corporation (CHC) have built RAIN (Resilient AI Network), an AI-powered network designed to help hospitals deliver and manage care anywhere.
The collaboration and technology were recently highlighted by Modern Healthcare, recognizing Resilient and CHC’s work to bring new models of care delivery and healthcare technology to community hospitals.
The technology publication Architecture & Governance Magazine also covered RAIN in an article on how AI may transform health care, written by Javier E. David, a veteran business journalist who has covered markets, finance, and the global economy for over 20 years.
In the article, David reports that clinicians who opt in “are then vetted by an administrative team before care is provisioned,” a step CHC’s Brian Doerr described as a deliberate choice to keep a “human in the loop” rather than let the algorithm make the final call. David also notes that “the system currently handles about 4,000 visits a month, with patients typically seen two to three times weekly.”
RAIN was developed as the technology infrastructure behind Resilient’s tech-enabled management services organization (MSO), helping coordinate the people, workflows, and clinical operations required to move care beyond the traditional four walls of a hospital or clinic.
Its immediate use case is outpatient physical therapy, with the platform supporting the process from referral and clinician matching through scheduling, care coordination, and clinical documentation.
“Healthcare has historically been organized around a location: the hospital, the clinic or the physician’s office. We built RAIN around a different idea: care should be able to happen anywhere the patient is,” said Dr. Jackleen Samuel, founder and CEO of Resilient Healthcare. “Technology gives us the ability to connect hospitals, clinicians, and patients in a completely different way and build an infrastructure that can scale with them.”
Built With Clinicians, Not Just for Clinicians
One of RAIN’s primary areas of focus is a longstanding challenge in healthcare: clinician burnout driven in part by documentation and administrative burden.
Clinical note-taking has historically been cumbersome and time-consuming, often requiring clinicians to complete documentation outside of patient care hours. For physical therapists in particular, that administrative workload can directly affect work-life balance.
For Samuel, a Doctor of Physical Therapy, understanding that experience firsthand became part of RAIN’s development process. Over the past year, she returned to treating patients while simultaneously helping develop the platform.
“I went back to treating because I wanted to put myself back in the shoes of the clinician,” Samuel said. “I wanted to understand where their time was actually going, what was frustrating and what technology could meaningfully take off their plate. We recognize clinician burnout is real, and if we are going to ask clinicians to use technology every day, it should make their lives better, not create another administrative burden.”
RAIN uses AI to organize patient information, schedules, and documentation in one place, making it easier for clinicians to prepare for visits without digging through PDFs or scattered records. By streamlining both the clinical and administrative parts of their day, RAIN is designed to combat clinician burnout and give therapists back time that would otherwise be spent managing schedules, searching for information, and completing notes after hours. That time can instead go to patients and their families.
Brian Doerr, SVP of Information Technology & Security and Privacy Officer at Community Hospital Corporation, said the collaboration reflects CHC’s focus on applying technology to meaningful operational challenges facing hospitals and clinicians.

Building the Infrastructure for Care Anywhere
RAIN is designed to support Resilient’s broader model of helping hospitals extend their clinical capabilities into the communities they serve.
Outpatient physical therapy is the first scaled application, but the underlying infrastructure is designed to support additional healthcare services as Resilient and CHC continue developing their partnership.
For community hospitals, that creates an opportunity to expand access without requiring every patient interaction to occur inside a traditional facility.
